Can somebody tell me about the districts in The Hunger Games?

I need to know what the books says about the physical geography of each district (mountains, hills, rivers, etc.) and where that suggests they are on a map of North America. I know that District 12 is in Appalachia and the Capitol is in the Rockies, but other than that I don't know. Tell me what information the book gives and give me quotes if you can, please.
